U. S. Citizenship DUTIES:
The Sioux Falls VA Health Care System is currently recruiting for a full-time , permanent, Social Worker (! HCHV-SUD), Health Care for Homeless Veterans, (HCHV), Substance Use Dis! order (SUD) program specialist. The social worker is under the direct supervision of the Homeless Coordinator. This position will be located in the Mental Health Service Line. As a Social Worker (HCHV-SUD), you are responsible for the following duties to include, but not limited to:

Provide direct SUD services for the HCHV program in the Medical Center and in the community. Provides Outreach and treatment services for the HCHV program in the community specifically for persons with substance abuse issues.

Provides training for other members of a multidisciplinary treatment team in areas related to SUD assessment, SUD with co-occurring disorders. Independently conducts intake and needs assessment of homeless Veterans and their significant others. Identifies symptoms leading to clinical diagnoses of SUD. Provides all necessary assessment functions, e.g. interviewing, psychosocial histories, and SUD assessments to aid in the development of treatment plans.
Develops comprehensive treatment plans with the Veteran that address the factors that contributed to homelessness, including SUD and other co-occurring factors within a recovery model framework. The recovery model includes developing harm reduction interventions. Works with the Veteran to realize the VeteranÂ's treatment goals, and reassesses and revises goals as indicated by the VeteranÂ's engagement in treatment. Organizes and conducts individual and group case management and SUD treatment, to include early interventions and supportive follow-up to ensure stability. Provides innovative treatment alternatives for those who have chronic SUD.

Provides SUD support to the HCHV team as a functional member of the team, to include consultation, interventions, or accompanying other HCHV personnel into the community. Will serve as a member of a multidisciplinary homeless program treatment team. Develops partnerships with community agencies, especially those serving h! omeless and SUD Veterans, with regular contact and communication.
Provide liaison services with other programs referring homeless Veterans to HCHV to ensure continuity of care and promote successful transition of Veterans from one program to another. Provide consultative services as needed.

Will participate in policy making, goal setting, and treatment service planning discussions with other members of the Homeless Programs. Provides recovery-oriented treatment with the goal of establishing the Veteran independently in the community at the VeteranÂ's highest level of functioning and quality of life. Develops programs and interventions for those who have chronic substance abuse and provides treatment for this population.

When assigned a social work student, incumbent will supervise training activities of the student and will adhere to VHA Handbook 1400.0 Supervision of Associated Health Trainees and VISN Documentation Policy 10N2-115-08.

The incumbent will enter all Veteran/family contacts in the electronic r! ecord using appropriate formats and templates. The incumbent is accurate, literate, and timely in charting, correspondence, and reports. Incumbent will judge effectiveness of techniques and when indicated seeks advice from the clinical treatment team or adjusts practices if therapies seem ineffective.

Maintain records and statistics required by the assignment, as well as any other information which may be needed for special studies and reports. Participates in staff development and training activities, research, program planning and development.

Assumes responsibly for professional development by various venues such as: Education of current theory and treatment modalities, reading professional journals, attending conferences, clinic seminars and teleconferences, and utilizing materials provided by Sioux Falls Health Care System (SFVAHCS).

Maintains a level of productivity and quality consistent with the complexity of the assignment and consistent ! with Social Work standards, the Joint Commission standards and facility! standards. Demonstrates knowledge and skill in the use of software applications for drafting documents and data management as well as other computer systems in use by the VHA.

Collaborates in the performance improvement processes and complies with performance measures. Serves on committees, work groups, and task forces at the facility as deemed appropriate by immediate supervisor and Social Work Executive.

Provides documentation of HCHV intervention (program overprints, progress notes, social summaries, addendum, consultations, etc.) that is done for each Veteran seen in the program. Documentation will provide a comprehensive description of the HCHV activities. Complete administrative and statistical reports to comply with program and administrative requirements. All reports are prepared and submitted within the time period specified and are accurate and complete.

Will give evidence of having regular contact and interaction with a variety of comm! unity resources and agencies.

The incumbent assumes responsibility for independent professional functioning and regularly works with a variety of complex cases within the assigned areas requesting consultation in unusually difficult situations and problematical decisions.

The incumbent is accountable for keeping the Homeless Coordinator/designee informed of administrative matters, professional/clinical issues and identifying problem areas that should be brought to the attention of the Coordinator. The Coordinator/designee through regular individual conferences and random record audits will review work.

Other duties as assigned.

Work Schedule: The tour of duty for this position is typically administrative hours, Monday-Friday, however, this position requires travel to outlying areas and may have irregular tours of duty; flexibility in tours and days of week will be required. Services may be provided to other Community Based Outpatie! nt Clinics (CBOC's). Travel will be required .

Relocation E! xpenses: Relocation expenses are not authorized for this position.

QUALIFICATIONS REQUIRED:
Basic Requirements:
Citizen of the United States .

Licensure : Be licensed or certified at the Master's level to independently practice social work.

English Language Proficiency : Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, Chapter 3, section A, paragraph 3j.

Education : Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to the CSWE website (Council on Social Work Educatio! n to verify if that social work degree meets the accreditation standards for a master of social work.

Exception : VA may waive the licensure and/or certification requirement for persons who are otherwise qualified, pending completion of state prerequisites for examination. VA social workers who are not licensed at the time of appointment must be licensed or certified at the Master's level, within 3 years of their appointment as a social worker. Those who fail to obtain state licensure or certification within the required time frames must be removed from the General Schedule (GS)-185 social worker series. This may result in termination of employment.

GR ADE DETERMINATIONS :
GS-9 : The GS-9 is the entry level grade for the GS-185 social work series and is used for social workers with less than one year of post-MSW experience and for social workers who are not yet licensed or certified at the independent practice level. Social workers! at the GS-9 level are working toward completion of prerequisites for l! icensure or certification. Applicants eligible for the GS-9 level must meet all basic requirements listed above. In addition, the candidates must demonstrate the following knowledge, skills, and abilities:

Ability to provide psychosocial treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational, and other diversified backgrounds. This requires knowledge of human development and behavior (physical and psychological), and the differential influences of the environment, society, and culture.

Ability to work with patients and families who are experiencing a variety of psychiatric, medical, and social problems utilizing individual, group, and family counseling skills. Work with more complex problems is done under close supervision. With guidance from the social work supervisor, ability to assess the psychosocial functioning and needs of patients and their family members, and to formulate and implement a treatment plan! , identifying the patientÂ's problems, strengths, weaknesses, coping skills, and assistance needed.

Basic knowledge of psychosocial treatment modalities and, under supervision, ability to implement treatment modalities in working with individuals, families, and groups to achieve treatment goals. This requires judgment and skill in utilizing supportive, problem solving, or crisis intervention techniques.

Ability to establish and maintain effective working relationships with clients, staff, and representatives of community agencies.

Ability to communicate effectively, both orally and in writing, with people from varied backgrounds.

5. Knowledge of medical and mental health diagnoses, disabilities, and treatment procedures. This includes acute, chronic, and
traumatic illnesses/injuries; common medications and their effects/side effects; and medical terminology.

6. Basic skill in the use of computer software applications for draf! ting documents, data management, and tracking. Ability to learn
and utilize software programs in use by VHA.

GS-11 : In addition to meeting all basic requirements, 1 year of professional social work experience, equivalent to the next lower grade level, under qualified social work supervision. This experience must have been in a clinical setting and must have demonstrated the potential to perform advanced assignments independently.

OR

In addition to meeting basic requirements, a doctoral degree in social work from a school of social work. A doctoral degree in social work from a school of social work may be substituted for the required 1 year of professional social work experience in a clinical setting.

Applicant must be licensed or certified at the Master's level to independently practice social work to be eligible for a GS-11.

In addition, the candidate must demonstrate the following knowledge, skills, and abilities:

Knowledge of community resources, how to make appropriate ! referrals to community and other governmental agencies for services, and ability to coordinate services.

Ability to independently assess the psychosocial functioning and needs of patients and their family members and to formulate and implement a treatment plan, identifying the patientÂ's problems, strengths, weaknesses, coping skills and assistance needed, in collaboration with the patient, family and interdisciplinary treatment team.

Ability to independently conduct psychosocial assessments and provide psychosocial treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds. Ability to independently provide counseling and/or psychotherapy services to individuals, groups and families. Social workers must practice within the bounds of their license or certification. For example, some states may require social workers providing psychotherapy to have a clinical level of licensure.

Ability to provide consultation services to other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. Ability to provide orientation and coaching to new social wo! rkers and social work graduate students. Ability to serve as a field instructor for social work graduate students who are completing VHA field placements.

Ability to independently evaluate his/her own practice through participation in professional peer review case conferences, research studies, or other organized means.

Knowledge and skill in the use of computer software applications for drafting documents, data management, and tracking, especially those programs in use by VHA.

Non-Qualifying Experience : Clinical social work experience obtained prior to completion of the requirements for a Master's Degree in Social Work does not satisfy this requirement. NOTE : A clinical setting is a medical center, clinic (medical or psychiatric), a residential treatment center, or any other type of facility where social work participates in collaborative treatment and is identified with the medical profession.

Grade Determinations :
! Note : The full performance level of this vacancy is 11. The act! ual grade at which an applicant may be selected for this vacancy is in the range of 09 to 11. The Professional Standards Board (a peer-review group) will review the selected candidateÂ's education and experience and recommend the grade and step at which the candidate will be placed. The salary is based on the grade and step approved for the selected candidate.

Intermittent complicated issues with Veteran or care giver may periodically become stressful. Docum! entation in electronic medical record is required for position necessitating good hand/eye coordination. Occasional lifting, moving, or transporting of VeteranÂ's possessions weighing up to 40 lbs. may occur. Community visits will require the ability to drive a vehicle. Must possess a valid driver's license.
